Discovered Spontaneous Lumbar Spinal Epidural Hematoma after Caudal Epidural Block in Patients Undergoing Anticoagulant Therapy. A Case Report

Abstract
Spinal epidural hematoma (SEH) is rare, but can lead to serious neurological deficits. However, because the initial symptoms of SEH, such as cervical or back pain, may be nonspecific, the hematoma or symptoms may be exacerbated if interventional procedures are performed to control pain. Therefore, distinguishing whether the cause of hematoma is spontaneous or iatrogenic is very important. Magnetic resonance imaging can detect changes in hematomas over time, and may help differentiate the cause. The authors performed a caudal epidural block to treat suddenly aggravated lower back and right lower limb pain and cauda equina syndrome in a 68-year-old man. Magnetic resonance imaging revealed acute epidural hematoma of the lumbar spine. This report documents the case and provides a brief review of the relevant literature.
